Membership & Pricing
Revive Health is a monthly healthcare membership that focuses on investigative and results-oriented medicine, without compromise. It is comprehensive and personal care without the middleman. For a low fixed monthly fee (less than the price of an office visit), patients get unlimited visits throughout the year and convenient access to their doctor via the electronic health record.
As insurance plans are becoming increasingly limiting year after year, Dr. Tiffany has made the decision to opt out of all insurance contracts.
She has done this for your benefit as she no longer wants to fight the battle of insurance companies dictating what she can and cannot do for you. She has chosen not to accept insurance so she can work directly for you (instead of constantly fighting constraints by the insurance juggernaut). This allows for greater availability, extended appointment times, price transparency, exceptional care, and many other benefits. In addition, eliminating a billing department enables her to pass those financial savings on to you. Dr. Tiffany will be accepting most credit cards, debit cards, and FSA/HSA cards for all services at Revive Health. All orders (labs, prescriptions, imaging, etc.) can still be submitted through insurance.
There is no one size fits all medicine.
When the hurdles to obtain comprehensive care are removed, being proactive with your health is that much easier.
Dr. Tiffany believes that everyone benefits from patient care that is tailored directly to them, whether it’s for optimization, prevention, or working towards improving acute or chronic conditions. This model allows Dr. Tiffany to fully practice that approach.

Membership Fees
$349 in-office or virtual (60 minutes)
New Patient Visit:

(even though Dr. Tiffany still recommends having at minimum a catastrophic health insurance plan)
Extended Visit Times
60 minute first office visits, 30-45 minute follow-up visits. Longer visits = more comprehensive care.
Unlimited virtual and in-office visits
Dr. Tiffany offers unlimited virtual and in-office visits with your membership. (BioTE visits are still a billed visit.)
Price Transparency.
No Copays…ever.
Unlimited messages to your doctor
via your electronic health record (i.e. you get additional access to your doctor with no added fees. It’s like having a doctor in your pocket!)
35% off all supplements
Dr. Tiffany doesn’t believe in making money off of supplements. Rather, she believes in getting you access to the components you need to optimize your health and extending that discount to you.
No insurance restriction
As a result, if you are going through a job transition, your care doesn’t stop!
No third party services or billing
All of our billing is done directly through our office at Revive Health.
Discounted and whole sale pricing
This includes labs, imaging, and prescriptions are available.
Cancel your membership anytime
We just ask patients provide written notice 30 days prior to the preferred date of cancellation or before the next billing cycle. Additionally, due to the limited number of memberships available, re-enrollment is not guaranteed once discontinued.
